Savannah, GA – THA Group’s Island Health Care was selected to participate in the Center for Medicare & Medicaid Services (CMS) Bundled Payments for Care Improvement (BPCI) initiative, a program made up of four different models with varying payment and care structures that include financial and performance accountability for episodes of care.
These models are anticipated to lead to higher quality, more coordinated care at a lower cost to Medicare by providing incentives for physicians, hospitals, and post-acute providers working collaboratively across all settings and specialties. Providers can track their patients’ care after they leave the hospital or doctor’s office, producing results more quickly than other innovative payment structures such as accountable care organizations.
Island Health Care, in partnership with Medtronic’s Cardiocom offering, began its BPCI pilot on January 1, 2015, with five anchor conditions: acute myocardial infarction (AMI), coronary artery bypass graft (CABG), cardiac defibrillator, percutaneous coronary intervention (PTCI), and diabetes. Four more anchor conditions will be added in April 2015 – cardiac valve, chronic obstructive pulmonary disease (COPD), bronchitis, asthma, congestive heart failure (CHF), and simple pneumonia and respiratory infections.
Island Health Care has practiced telehealth since 2001, and telemonitoring is the standard of care. Every bundled patient will be monitored by Cardiocom and THA Group’s RightHealth® telehealth centers for 24 hours a day for each 90 day episode. Upon discharge, bundling partners will work closely with physicians to keep patients safe at home through the use of the described technology, thus reducing expensive medical costs.
